The BPS CGC Enrollment
Package includes:
- BPS CGC
Participation and Confidentiality Agreement. The
confidentiality agreement between PEP and each participating
company will protect all information shared and
developed during the project. Please sign and return to PEP.
- Working Group
Contact Information. Please provide the contact
information for the point person for the project, as well as
any additional environmental or technical staff that will be
involved in the meetings.
- Invoice:
The 2009 fee for BPS CGC is $5000 for companies with 100+
employees. The fee for companies with 99 or less employees
is $3500.
After the enrollment
forms have been received, companies will receive an excel
spreadsheet to begin initial data gathering in anticipation of the
first working group meeting.
Please note:
All forms and payment must be received in the PEP office
before your company can attend working group meetings. The
first working group is tentatively scheduled for May 2009.
